About the FON Data Collection Forms
The Fontan Outcomes Network has made its enrollment data collection forms available upon completion of the brief form below in order to transparently share our data collection strategy.
These data collection forms will be used to enter patient data into our registry. Please note that the version currently available reflects the data collection forms as of registry launch on October 18, 2022, and the forms may change as the registry evolves and we learn about data collection processes at our participating Care Centers.
