Active Quality Improvement Projects

Our team of researchers, patients, parents, cardiologists, nurse practitioners, psychologists designed the system level diagram for improving outcomes for individuals with Fontan circulation. The team outlined many opportunities, which are visually displayed on the System Level Diagram to the right. Then, via a survey/interview process and feasibility/readiness assessment, the first two improvement projects were selected: Physical Activity and Exercise and Liver Health.

  • Project Phase:

    The Physical Activity and Exercise Project is in the planning and design phase.

    Brief Summary:

    FON is driving a transformative initiative for individuals with Fontan circulation by promoting active lifestyles. In response to declining exercise capacity and quality of life, the development of the Active Living with Fontan Circulation toolkit is underway. Preliminary data indicates its readiness for refinement and broader testing.

    SMART Aim:

    As our project is in the planning and design phase, we are in the process of considering our SMART Aim.

    Participation Details:

    The Physical Activity and Exercise Project is currently a closed group. As the project progresses, there may be opportunities to get involved. Stay tuned!

    Brief Summary:

    In surveys of FON care centers and individuals with Fontan circulation and their families, Fontan-associated liver disease (FALD) was identified as a priority area on which to focus QI efforts. This project aims to improve the reliable liver health education, surveillance and management of individuals living with Fontan circulation.

    SMART Aim:

    As our project is in the planning and design phase, we are in the process of considering our SMART Aim.
