Welcome to the FON and NPC-QIC Spring 2025 Virtual Learning Session!
Our theme for this learning session is Transitions, focusing on the transition to SV-ONE and our evolving lifespan approach as a joint network.
Our Global Learning Session Objectives are:
Identify recent and upcoming activities and accomplishments within NPC-QIC and FON
Describe current NPC-QIC and FON improvement projects and data analytics
Discuss topics across the lifespan that impact single ventricle care and outcomes
Build community with patients, parents and healthcare professionals and develop a team 90-day action plan

Zoom details and additional session information will be shared via email with registered attendees in the weeks leading up to the event. You must register to receive login information.
Zoom Etiquette Reminders
To ensure a smooth session experience for all attendees, please keep the following in mind:
We encourage you to find a quiet room, free from distractions, to participate in the Virtual Learning Session.
Change your Zoom name to feature your name and title, or your care center name if you join the session as a group.
Using a Zoom background highlighting your care center or organization is recommended.
Please use the 'raise your hand' feature and wait to be called on before speaking. Remain muted when not speaking.
We encourage you to introduce yourself before speaking or asking questions.
While the agenda includes time for breaks, we encourage you to turn off your camera and microphone to excuse yourself for a break as needed.
Refrain from writing any protected health information (PHI) or private details about yourself or others in the chat box.
If the Zoom call suddenly ends, please check the email address you used for registration for instructions on re-joining the session.

Post Event Survey
Our Community Building & Engagement Workgroup has created a Post Virtual Learning Session Survey to learn about your experience and make improvements for future learning sessions. This brief 5 question survey for patients and families takes approximately 5-7 minutes to complete. Respondents who submit their survey by June 6 will be entered into a drawing for the Sisters by Heart Warrior Wednesday feature! Take the survey here!
Join the Single Ventricle Patient Day Planning Committee
We are looking for a new member to join the Single Ventricle Patient Day Planning Committee! The role involves monthly virtual planning meetings to organize the next Single Ventricle Patient Day, which we aim to hold in the fall alongside the Fall Learning Session. Any individual aged 14+ with single ventricle heart disease is welcome to apply by May 28! Click here for details!

The agenda includes two Team Time sessions for discussion with your care center team: the SV-ONE Transition Team Time at 12:05 p.m. ET, and Self-Guided Reflection & Planning Team Time session at 4:05 p.m. ET.
Please prepare for Team Time by:
Reserving a meeting space for collaboration or planning a virtual meeting for both sessions (12:05-12:30 p.m. ET and 4:05-5 p.m. ET).
Downloading a copy of the Team Time worksheet or having printed copies available for your team to use during the SV-ONE Transition Team Time at 12:05 p.m. ET
