Are you a care center looking to improve outcomes for individuals with single ventricle congenital heart disease? We are excited to invite your care center to apply to join the Fontan Outcomes Network (FON)! 

What is FON?

FON is a lifespan learning health network featuring a community of patients, families, clinicians, and researchers dedicated to optimizing physical health, achieving ongoing developmental surveillance, and increasing emotional health and resilience for individuals with Fontan circulation. 

The Benefits of Participating in FON

While the primary motivation for your care center’s participation in our network will be to improve care and outcomes for patients with single ventricle heart disease, FON will offer additional benefits for your center, including: 

  • Access to the lifespan registry data, with more than 900 patients enrolled

  • Authorship opportunities to publish using more granular registry data 

  • Faculty and staff opportunities to serve on various workgroups and committees

  • Participation in bi-monthly case review conferences

Additionally, a care center's participation in FON may soon come to be viewed by patients and families as a deciding factor in where to receive their ongoing care.

Application Process and Onboarding Sessions

To apply to become a FON care center, review this application packet and complete the online care center application by June 16, 2024

FON will also host two onboarding webinars for new care centers. If you plan to complete the application, please hold one of the following dates on your calendar and use the links below to register:
